Enrollment Form

It is mandatory for a customer to provide his details at the time of enrollment. The DSE/ Loyalty Champion will enter the details in the CRM system. All the fields marked in RED should be filled. Most important fields are:

  • Insta Loyalty Number : Insta number will be entered by Loyalty Champion/ DSE in system of the kit handed to customer.
  • Name on the card: Name printed on the card would be taken from this field so it should be filled correctly
  • Address and Contact details: The welcome kit would be mailed on the details shared in this field so correct details should be shared. If possible POSTAL CODE should be shared so that remote destinations can be covered
  • Date of Birth, Mother’s Name & Place of Birth becomes questions for identification verification in case of loss of card. These details would be asked by customer contact centre while taking a request to issue duplicate card
  • Signature and Stamp both from Dealer representative and Customer should be captured

Chassis Form

The number of Chassis entry entered is equivalent to number of TATA commercial vehicles registered in the program. The Chassis number of vehicle should be captured along with signature, stamp.
